How to enroll your student...Go To Top of Page

Step One - complete the forms below (click the links to download and print the forms)

Step Two - gather all the documents you must provide at time of enrollment

1. Proof of Residence. Under Education Code section 48204.1, the following documents establish proof of residency in an attendance zone:
  • Property tax payment receipts;
  • Rental property contract, lease, or payment receipts;
  • Utility service contract, statement, or payment receipts;
  • Pay stubs;
  • Voter registration;
  • Correspondence from a government agency; or
  • Declaration of residency executed by the parent or legal guardian of the pupil.
2. Immunization Record (California State Law requires this and must be provided for attendance).
 
3. Proof of Age. Under Education Code section 48002, the following documents establish age:
  • Certified copy of a birth record (if this is not available, you may submit one of the following).
  • Statement by the local registrar or a county recorder certifying the date of birth;
  • Baptism certificate;
  • Passport; or
  • Affidavit of the parent, guardian, or custodian of the minor.
4. If your student is receiving special education services or 504 accommodations, a copy of the most recent IEP or 504 plan must accompany this packet.
 
In Addition, if the student is a current high school student we need:
 
  • A current transcript.
  • Exit grades from previous school (if enrolling during the school year).
  •  
    Other Documents:
    • If you have a restraining order against any person, involving this student, please present this restraining order so that a copy can be placed in the student’s records.
    • Please provide a copy of any Custodial papers pertaining to your student at the time of registration.  We can only enforce parental rights with legal documentation.
    • Please provide a copy of foster placement or guardianship papers.
    Students must be registered under their full legal name.
     
    Step Three - Call for an appointment with the Registrar
    • Call Regina Medina 530.622.3634 x1029 to make an appointment to register
    • Bring all completed forms and required documents with you to the appointment
    • Important information about your registration appointment:

    1. Student MUST be registered under his/her full legal name.
    2. Any information, with court documentation, indicating custody arrangements, restraining order, no contact, limited contact with non-custodial parent/grandparent etc, should also be presented at the meeting.

    You must make an appointment with your counselor BEFORE checking out. Please call 530.622.3634 x1029 or see Mrs. Medina to make an appointment.

    Things to keep in mind for your appointment...

    • If you call for your appointment 72 hours in advance, your transcript with withdrawal grades (which you will need for registration at your new school) will be available at your appointment. Without sufficient notice, your transcript will be delayed.
    • Text books, library books, ID card, sports uniforms, and other borrowed items must be returned to the school at the time of check out.
    • All fees and fines must be paid in order to obtain the student's official transcript.
    • The new school's address and phone number is needed upon checking out.
    • Parent/guardian must be present at the time of check out.

    To order a copy of your transcript, please complete and print out the transcript request form.
    (Or if you prefer, stop by the Counseling Office between 7:30 am - 4:00 pm to complete the form.)
     
    Please note: No fee will be charged to current students. Former students may request up to two transcripts at no cost.  Each additional transcript after the first two free, will be subject to a $1.00 fee per transcript (postage rates may apply).
     
    Please mail the completed transcript request form, along with $1.00 per transcript (if ordering more than two transcripts) to:

    Checks will not be accepted.  Cash or money order only. 

    Once the transcript request is received in our office, it will take 24 hours to process.

    Diploma RequestsGo To Top of Page

    To order a new high school diploma, please complete and print out the diploma request form.
    (Or if you prefer, stop by the Counseling Office during normal business hours to complete the form.)

    Mail the completed form, along with a $25 money order made payable to "Herff Jones" to the following address:

    El Dorado High School
    Attn: Registrar
    561 Canal Street
    Placerville, CA 95667

    Once the diploma request is received in our office, it will be sent to Herrf Jones and will take 6-8 weeks to process.
