Step One - complete the forms below (click the links to download and print the forms)
1. Proof of Residence. Under Education Code section 48204.1, the following documents establish proof of residency in an attendance zone:
- Property tax payment receipts;
- Rental property contract, lease, or payment receipts;
- Utility service contract, statement, or payment receipts;
- Pay stubs;
- Voter registration;
- Correspondence from a government agency; or
- Declaration of residency executed by the parent or legal guardian of the pupil.
2. Immunization Record (California State Law requires this and must be provided for attendance).
3. Proof of Age. Under Education Code section 48002, the following documents establish age:
- Certified copy of a birth record (if this is not available, you may submit one of the following).
- Statement by the local registrar or a county recorder certifying the date of birth;
- Baptism certificate;
- Passport; or
- Affidavit of the parent, guardian, or custodian of the minor.
4. If your student is receiving special education services or 504 accommodations, a copy of the most recent IEP or 504 plan must accompany this packet.
In Addition, if the student is a current high school student we need:
- A current transcript.
- Exit grades from previous school (if enrolling during the school year).
Other Documents:
- If you have a restraining order against any person, involving this student, please present this restraining order so that a copy can be placed in the student’s records.
- Please provide a copy of any Custodial papers pertaining to your student at the time of registration. We can only enforce parental rights with legal documentation.
- Please provide a copy of foster placement or guardianship papers.
Students must be registered under their full legal name.
Step Three - Call for an appointment with the Registrar
- Call Regina Medina 530.622.3634 x1029 to make an appointment to register
- Bring all completed forms and required documents with you to the appointment
- Important information about your registration appointment:
1. Student MUST be registered under his/her full legal name.
2. Any information, with court documentation, indicating custody arrangements, restraining order, no contact, limited contact with non-custodial parent/grandparent etc, should also be presented at the meeting.
